If you live in Indianapolis and surrounding areas and want to know if you qualify for additional assistance, the State of Indiana partners with the local nonprofit organization CICOA to check your eligibility for the Medicaid Waiver. To learn more about CICOA click below!

Here are a few help tips and resources to help you get started! Please reference this page for your convenience.
Step 1:
To apply for the Indiana A&D Medicaid Waiver, you will need to find your local Area Agency on Aging (AAA). Indiana FSSA has tasked these entities with managing the waiver application and utilization process. For Individuals that live in Marion County, Johnson County, Hamilton County, Hendricks County and Hancock County please contact CICOA. You can schedule an appointment here or you can submit a referral request here. You can also contact them directly at 317-254-5465.

If you live in other counties please visit the AAA to find your local agency to apply.

Step 2:
Congratulations! Once you have been approved for the Medicaid Waiver, you will be assigned a Medicaid number and will be connected to a Care Manager. Please visit here for more additional information about the navigation process once connected to CICOA.
Step 3:
Great News! You now will be able to select your preferred in-home care agency. We hope you keep us in mind when choosing your provider. If you would like us to be your service care provider please contact us to schedule an in-home observation to discuss your needs and develop your personal care plan. The client care plan is designed and implemented to ensure quality care by establishing a personalized plan customized to the client to determine the services requested and frequency of the requested services.
Step 4:
Lastly, after the care plan is established a caregiver will be assigned and services will start immediately! We strive to find the right caregiver just for you!
